Our 2010 SmartScribe Home Health Diagnoses form, the "CM-1" serves to capture both primary and secondary diagnoses in M1020 and M1022, case-mix diagnoses in M1024 as well as the level of symptom control as required for OASIS. Additionally, we have included optional fields for both date and onset/exacerbation for agencies that collect that data for their records.
